What size baby requires a C-section?

A planned C-section might be considered based on the estimated fetal weight, specifically if the baby is suspected to be very large (macrosomia). According to ACOG, the following fetal weight estimates may lead to a planned C-section:

Maternal Condition Estimated Fetal Weight Threshold
Diabetic 10 pounds or more
Non-Diabetic 11 pounds or more

It's important to note that ACOG states that ultrasound is no better than a provider's exam in estimating fetal weight. Suspected macrosomia should not be an indication for induction of labor. Planned C-sections are typically only considered when the estimated fetal weight reaches the thresholds outlined above.
