There isn't a single "most accurate GFR level" because the accuracy depends on the method used to measure it. The most accurate GFR measurement uses plasma or urinary clearance of an exogenous filtration marker. This is considered the gold standard. However, this method is complex and not always practical for routine clinical use.

Understanding GFR and its Measurement

Glomerular filtration rate (GFR) is a crucial indicator of kidney function. It measures the rate at which blood is filtered through the glomeruli (filtering units in the kidneys). A normal GFR varies depending on factors like age and body size, but generally, higher GFR values indicate healthier kidneys.

While direct measurement of GFR (using exogenous markers) is the most precise, it's often replaced by estimated GFR (eGFR) calculations in clinical practice. These calculations use readily available serum creatinine levels (a waste product of muscle metabolism) and other factors like age, sex, and race in formulas. The accuracy of eGFR varies depending on the formula used and individual patient characteristics.

Several eGFR equations exist, including the Modification of Diet in Renal Disease (MDRD) and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations. Studies show that the MDRD and CKD-EPI equations offer more accurate estimates than older methods, such as creatinine clearance alone. Even these equations, however, are estimates and are less accurate at higher GFR values. Moreover, in certain cases, combining creatinine and cystatin C levels can improve the eGFR estimate.

Interpreting GFR Results

The interpretation of GFR values typically uses stages to classify kidney function:

  • eGFR ≥ 90 mL/min/1.73m²: Generally considered within the normal range.
  • eGFR 60-89 mL/min/1.73m²: May indicate early-stage kidney disease.
  • eGFR 15-59 mL/min/1.73m²: Indicates kidney disease.
  • eGFR < 15 mL/min/1.73m²: Indicates kidney failure.
