The 5 Whys Technique Explained

  1. Define the Problem: Clearly state the problem you are trying to solve.

  5. Why 4: Why was the tablet press not calibrated correctly?

    • Answer: The calibration schedule was not followed.
  6. Why 5: Why was the calibration schedule not followed?

    • Answer: The technician responsible was not adequately trained on the new calibration procedure.
