A normal head assessment typically reveals a head that is upright, centered, and symmetrical, with no visible masses, swelling, deformities, or discolorations of the cranium or neck.
A comprehensive normal head assessment considers the following:
- Position: The head is held upright and centered on the body.
- Symmetry: The cranium and neck are symmetrical in appearance.
- Contour: The skull should have a smooth, rounded contour without any unusual protrusions or depressions.
- Masses, Swelling, or Deformities: There should be no palpable or visible masses, swelling, or deformities present on the scalp, skull, or neck.
- Discoloration: The skin of the scalp and neck should be consistent with the individual's overall skin tone, without any unusual redness, bruising, or other discolorations.
- Hair Distribution: The hair distribution should be even and appropriate for the individual's age and gender.
- Scalp Condition: The scalp should be clean and free from lesions, flaking, or excessive dryness.
If any asymmetry, masses, swelling, deformities, or discolorations are noted during the initial visual assessment, further palpation and evaluation are required to determine the underlying cause. These findings are then documented in detail.
