What is a grade 4 ulcer?

Published in Pressure Ulcer Stages 2 mins read

A grade 4 ulcer, also known as a stage 4 pressure ulcer, is the most severe form of pressure ulcer.

Understanding Grade 4 Pressure Ulcers

Grade 4 pressure ulcers represent significant damage and carry a high risk of complications. Here's a detailed look at their characteristics:

Key Features of a Grade 4 Pressure Ulcer

  • Severe Tissue Damage: The skin is extensively damaged and the tissue around it begins to die (necrosis).
  • Underlying Structure Involvement: Muscles, tendons, and even bones beneath the skin may be damaged.
  • High Infection Risk: Individuals with grade 4 ulcers have a significantly higher risk of developing severe, potentially life-threatening infections.

Severity Levels of Pressure Ulcers:

To understand the severity of a grade 4 ulcer, it's helpful to know the various stages:

Grade Description
1 Red, intact skin that does not turn pale when touched.
2 Partial thickness skin loss involving the epidermis or dermis. May present as a blister or shallow ulcer.
3 Full thickness skin loss extending to the subcutaneous tissue, showing visible fat, but not bone or muscle.
4 Full thickness skin loss with extensive tissue damage and necrosis, potentially involving muscle, tendon, or bone.

Risk and Complications

A person with a grade 4 ulcer is in a precarious health situation and are at high risk of:

  • Serious infections that can spread to the bloodstream (sepsis), which can be life-threatening.
  • Osteomyelitis: An infection of the bone, common when the ulcer reaches bone.
  • Delayed healing and long periods of treatment.

Why are grade 4 ulcers dangerous?

Grade 4 ulcers are dangerous because of the depth of the tissue damage. The damage can extend through multiple layers, such as:

  • Skin
  • Subcutaneous fat
  • Muscles
  • Bone

The necrosis (dying tissue) creates an ideal environment for bacteria to thrive. If these infections reach the bone, blood stream, or surrounding tissues, it can be extremely dangerous. The damage and infections are also incredibly difficult to treat.

Important Note

According to reference material dated October 16th, 2024, individuals with grade 4 pressure ulcers face a high likelihood of developing life-threatening infections due to the severity of the tissue damage and compromised immune system.
